The One-Leg Balance Test: A Simple Way to Check How Well You’re Aging

Aging is often measured by obvious milestones: how fast you walk, how strong your grip is, or how easily you climb a flight of stairs. But one surprisingly simple ability may reveal a great deal about how your body is changing: standing on one leg. You don’t need a fitness tracker, gym equipment, or a complicated medical test—just a few seconds of balance and a safe place to stand.

Research from Mayo Clinic suggests that single-leg balance may decline with age more noticeably than some other physical measures. In a study of healthy adults over 50, researchers found that standing on the nondominant leg showed the greatest age-related decline among the balance, strength and walking measures they tested. But this isn’t a crystal ball for longevity. Rather, your ability to balance offers a useful snapshot of several systems working together—and can provide an early reason to pay more attention to strength, mobility and fall prevention.

Why Does Standing on One Leg Matter?

Balance is much more complicated than it looks. To remain upright on one foot, your brain has to combine information from your eyes, inner ear and sensory nerves while coordinating muscles throughout your body. Mayo Clinic researchers describe balance as a measure involving visual, vestibular, somatosensory and neuromuscular systems.

Those systems can change as we get older. What the Research Says About the 10-Second Test

One of the most widely discussed findings comes from a 2022 study published in the British Journal of Sports Medicine. Harvard Health reports that adults ages 51 to 75 who could not stand on one leg for 10 seconds had almost twice the risk of premature death during the study’s follow-up period.

That sounds dramatic, but it is important to understand what the finding does—and does not—mean. The study showed an association, not proof that poor balance causes an earlier death. Cleveland Clinic similarly cautions that the 10-second test is not a perfect predictor of lifespan or a complete assessment of someone’s health.

In other words, failing to stand on one leg for 10 seconds does not mean you are destined for poor health. It may instead be a useful signal that balance, mobility or physical fitness deserves more attention.

How to Try the Test Safely

If you want to check your balance, don’t turn the test into a dare. Safety comes first.

Stand near a sturdy countertop, wall or chair that you can grab if necessary. Keep your eyes open and look straight ahead. Lift one foot from the floor and see how long you can remain balanced without putting it down or grabbing the support.

Harvard Health’s more recent guidance recommends performing balance tests near something stable or with another person nearby, particularly if you are unsteady. For the four-stage balance test, 10 seconds without support is used as a goal before moving to more challenging positions.

Mayo Clinic researchers have suggested that being able to stand on one leg for around 30 seconds is a strong performance, while their clinical guidance emphasizes that even practicing the movement can help train balance.

The exact number of seconds, however, shouldn’t become an obsession. Your age, fitness level, previous injuries and medical conditions can all affect your performance.

What If You Can’t Stand for 10 Seconds?

Don’t panic—and don’t assume that a short balance time means you’re “aging badly.” It simply means your balance may have room for improvement.

The good news is that balance is trainable. Harvard Health recommends practicing single-leg balance regularly, while Cleveland Clinic suggests exercises such as single-leg stands and tai chi to improve stability.

The NHS also includes one-leg stands in its balance exercises for older adults, recommending that beginners use a wall for support and gradually work toward holding the position for five to 10 seconds.

You can start by holding a counter with one or both hands, lifting one foot briefly, then gradually reducing your reliance on the support. As your confidence improves, you can increase the duration. Regular walking, lower-body strengthening and other balance exercises can complement this practice.

Balance Is About More Than Longevity

The most valuable reason to work on your balance isn’t necessarily to predict how long you’ll live. It’s to help preserve the ability to move through everyday life safely and independently.

Falls become a significant health concern as people age, and poor balance can contribute to falling. Mayo Clinic notes that age-related changes in muscles, joints and coordination can affect stability and increase fall risk.

A simple one-leg stand therefore offers a useful reality check: How steady do you feel? Is one side noticeably weaker than the other? Has your balance changed compared with a few years ago?

If you notice a substantial decline, frequent stumbling, dizziness or difficulty performing everyday movements safely, consider discussing it with a healthcare professional. A clinician or physical therapist can perform a more comprehensive balance assessment and identify potential causes.

The Bottom Line

Standing on one leg won’t tell you your biological age or predict exactly how long you’ll live. What it can do is provide a quick glimpse into an important aspect of physical aging.

If you can stand comfortably on one leg, that’s encouraging—but maintaining that ability requires continued movement. If you struggle, there’s no need to feel discouraged. Balance can be practiced and improved, often with simple exercises that require little or no equipment.

Perhaps the best lesson from the one-leg test is this: healthy aging isn’t simply about adding years to your life; it’s about maintaining the strength, coordination and confidence to use those years well.
